<p>There have been recent claims that dairy products can help people lose weight, and the dairy industry has hyped the assertion by investing millions of dollars in commercial advertising. However, a new review of the evidence published in the journal Nutrition Reviews reveals that neither dairy nor calcium intake promotes weight loss.</p>
<p>Amy Joy Lanou of the University of North Carolina at Asheville and Neal Barnard with the Physicians Committee for Responsible Medicine in Washington, DC, evaluated evidence from 49 clinical trials from 1966 to 2007 that assessed the effect of milk, dairy products, or calcium intake on body weight and BMI, with or without the use of dieting.</p>
<p>Evidence from the trials showed that neither dairy products nor calcium supplements helped people lose weight. Of the 49 clinical trials, 41 showed no effect, two demonstrated weight gain, one showed a lower rate of weight gain, and only five showed weight loss.</p>
<p>An association between calcium or dairy intake and weight loss seen in some observational studies may be attributable to other factors, such as exercise, decreased soda intake, lifestyle habits, or increased fiber, fruit, and vegetable intake.</p>
<p>&#8220;Our findings demonstrate that increasing dairy product intake does not consistently result in weight or fat loss and may actually have the opposite effect,&#8221; the authors conclude.</p>
<p>Study In 7,000 Men And Women Ties Obesity, Inflammatory Proteins To Heart Failure Risk</p>
<p>Heart specialists at Johns Hopkins and elsewhere report what is believed to be the first wide-scale evidence linking severe overweight to prolonged inflammation of heart tissue and the subsequent damage leading to failure of the body&#8217;s blood-pumping organ.</p>
<p>The latest findings from the Multiethnic Study of Atherosclerosis (MESA), to be published in the May 6 issue of the Journal of the American College of Cardiology, appear to nail down yet one more reason for the estimated 72 million obese American adults to be concerned about their health, say scientists who conducted the research.</p>
<p>&#8220;The biological effects of obesity on the heart are quite profound,&#8221; says senior study investigator João Lima, M.D. &#8220;Even if obese people feel otherwise healthy, there are measurable and early chemical signs of damage to their heart, beyond the well-known implications for diabetes and high blood pressure.&#8221;</p>
<p>He adds that there is &#8220;now even more reason for them to lose weight, increase their physical activity and improve their eating habits.&#8221;</p>
<p>In the latest study, researchers conducted tests and tracked the development of heart failure in an ethnically diverse group of nearly 7,000 men and women, age 45 to 84, who were enrolled in the MESA study, starting in 2000.</p>
<p>Of the 79 who have developed congestive heart failure so far, 35 (44 percent) were physically obese, having a body mass index, or BMI, of 30 or greater. And on average, obese participants were found to have higher blood levels of interleukin 6, C-reactive protein and fibrinogen, key immune system proteins involved in inflammation, than non-obese adults.</p>
<p>A near doubling of average interleukin 6 levels alone accounted for an 84 percent greater risk of developing heart failure in the study population.</p>
<p>The researchers from five universities across the United States also found alarming links between inflammation and the dangerous mix of heart disease risk factors known as the metabolic syndrome. Its combined risk factors for heart disease and diabetes - high blood pressure, elevated blood glucose levels, excess abdominal fat and abnormal cholesterol levels, and particularly obesity - double a person&#8217;s chances of developing heart failure.</p>
<p>&#8220;More practically, physicians need to monitor their obese patients for early signs of inflammation in the heart and to use this information in determining how aggressively to treat the condition,&#8221; says Lima, a doctor of radiology at the Heart Institute.</p>
<p>All MESA study participants, who will be followed through to 2012, had no pre-existing symptoms of heart disease. Upon enrollment, they all underwent a physical exam, including weight and body measurements, blood analysis and an MRI scan to assess heart function.</p>
<p>&#8220;Our results showed that when the effects of other known disease risk factors - including race, age, sex, diabetes, high blood pressure, smoking, family history and blood cholesterol levels - were statistically removed from the analysis, inflammatory chemicals in the blood of obese participants stood out as key predictors of who got heart failure,&#8221; says Lima.</p>
<p>The chemicals are all known to be part of the body&#8217;s defensive response to disease. They are well-recognized for producing symptoms that stem from the widening of small blood vessels, including redness and fever, and the release of immune system cells that make blood vessels leak fluid into surrounding tissue, causing swelling and cell death. The inflammatory process eventually leads to cell damage and the buildup of scar tissue near the damaged areas.</p>
<p>In obese participants, interleukin 6, a chemical that activates white blood cells and drives inflammation, was higher than in non-obese participants.</p>
<p>Similarly, a near tripling of average levels of C-reactive protein in study participants increased the chance of heart failure by 36 percent.</p>
<p>C-reactive protein levels are widely known to rise dramatically and speed up the early stages of inflammation when cells swell up with fluid, leading to widespread arterial damage.</p>
<p>One-fifth higher than average blood levels of fibrinogen, best known for its role in blood clotting but also a major player in muscle scarring, bumped up the risk of heart failure by 37 percent.</p>
<p>When the inflammatory protein levels were included in the scientists&#8217; statistical analysis, the heightened risk from obesity disappeared.</p>
<p>&#8220;What this tells us is that both obesity and the inflammatory markers are closely tied to each other and to heart failure,&#8221; says lead researcher Hossein Bahrami, M.D., M.P.H.</p>
<p>Each year, nearly 300,000 Americans die from heart failure.</p>
<p>Bahrami says study results also point to inflammation as a possible catalyst in metabolic syndrome. Increased blood levels of albuminuria, a chemical more known for its association with impaired kidney function and metabolic syndrome boosted risk of a progressively weakening heart nearly tenfold among MESA participants.</p>
<p>Bahrami, a senior cardiology research fellow at Hopkins, says &#8220;the basic evidence is building the case that inflammation may be the chemical route by which obesity targets the heart, and that inflammation may play an important role in the increased risk of heart failure in obese people, especially those with the metabolic syndrome.&#8221;</p>
<p>He notes that previous studies, also done at Hopkins, have shown that even moderate exercise to lose abdominal fat dramatically offsets the harmful effects of metabolic syndrome on heart function.</p>
<p>Bahrami says the team&#8217;s next steps are to determine how, over a longer timeframe, heart function changes with levels of inflammatory markers, and to see if alterations to the immune system proteins halts or speeds up disease.</p>
<p>BMI is calculated from a person&#8217;s weight and height and serves as a good indicator of overall body fatness (where a person&#8217;s weight in kilograms is divided by the square of height in meters; or using weight in pounds, divided by the height in inches squared, multiplied by a metric conversion factor of 703.)<br />

The truth, experts say, is that even when you&#8217;re &#8220;on a diet,&#8221; you may be eating a lot more calories than you think. There&#8217;s often a disconnect between what we know we should do to lose weight, and what we actually do while trying to diet.</p>
<p>For starters, stop thinking about dieting. Instead, take a look at those everyday habits that could be causing weight gain. Going on a diet can create an obsession with food, heighten cravings, and lead to a &#8220;throw-in-the-towel-because-diets-don’t-work&#8221; mentality.</p>
<p>You might not realize just how quickly calories can add up. An extra tablespoon of salad dressing can add 75-100 calories, an extra tablespoon of butter adds 102 calories, and that 1-ounce bag of chips with your sandwich at lunch adds 162 calories. Eating while cooking, starting each day with a high-calorie coffee drink, finishing off the kids&#8217; plates at dinner, or having one too many glasses of wine &#8212; these are just a few of the sneaky habits that sabotage weight loss efforts.</p>
<p>Yet as quickly as calories can add up, they can be subtracted. Becoming mindful of your diet mistakes &#8212; the subtle ways that calories sneak into your diet throughout the day – can add up to real weight loss.</p>

<p>Diet Mistake No. 1: Racing to the Finish<br />
There&#8217;s no reward for finishing your meal in record time &#8212; unless you&#8217;re a contestant in a hot dog eating contest! Our hectic schedules have led many of us to adopt the unhealthy habit of rapid eating.</p>
<p>&#8220;We need to adopt more of the leisurely, European-style eating so that we can savor our food, taste every bite, and get the signal of fullness before overeating,&#8221; says Tara Gidus, MS, RD, a spokeswoman for the American Dietetic Association.</p>
<p>Diet Mistake No. 2: Skipping Meals<br />
Research shows that breakfast skippers weigh more than breakfast eaters. There is a misconception that skipping breakfast &#8212; or any meal &#8212; saves calories. The truth is that most people who eat fewer than three meals usually end up eating more calories during the course of the day.</p>
<p>Strive for three meals a day. Always start your day with a healthy breakfast, but be careful to choose wisely.</p>
<p>&#8220;Even a low-fat muffin can have as many as 400 calories and 5 grams fat,&#8221; says Joanne Lichten, PhD, RD, a nutrition consultant and the author of Dining Lean.</p>
<p>A healthy breakfast should contain both protein and fiber. An egg, a piece of whole-wheat toast, and half a grapefruit has only 250 calories and will keep you feeling full until lunch.</p>
<p>Diet Mistake No. 3: Too Many Liquid Calories<br />
Liquid calories from alcohol, smoothies, coffee with cream and sugar, sweetened juices, teas, and sodas can really contribute to weight gain. One recent study found that Americans get approximately 21% of their calories from beverages. &#8220;When you drink beverages, you don’t tend to compensate by eating less because most beverages satisfy thirst and don’t impact hunger,&#8221; says Gidus.</p>
<p>Switch from calorie-laden beverages to water, club soda, skim milk, vegetable juices, and small portions of 100% fruit juice. If you drink alcohol, do so in moderation, and choose lighter drink options.</p>

Diet Mistake No. 4: Oversized Portions<br />
&#8220;We have gotten used to huge portions at restaurants so when we are at home, we serve up the same size and think it is normal,&#8221; says Lichten.</p>
<p>Experts suggest a few tricks to help you trim your portions:</p>
<p>Leave a few bites on your plate.<br />
Use smaller plates and bowls.<br />
Periodically check your portions with measuring cups.<br />
Diet Mistake No. 5: Choosing Unhealthy Add-Ons<br />
Not only have portions crept up in size, we also have a tendency to top off our &#8220;diet&#8221; salads and other favorite foods with high-fat toppings, like bacon, cheese, croutons, and creamy dressings.</p>
<p>And, at fast-food restaurants, &#8220;grilled chicken and salads are not always better than a burger,&#8221; notes Lichten. &#8220;It all depends on the size and the toppings.&#8221;</p>
<p>For example, the Burger King Tendergrill sandwich with honey mustard dressing has 450 calories while their Whopper Jr., with mustard instead of mayo, has only 290 calories. At McDonald’s, the Caesar salad with crispy chicken and creamy dressing totals 490 calories, while a Quarter Pounder weighs in at 410 calories.</p>
<p>Diet Mistake No. 6: Mindless Eating<br />
&#8220;Eating amnesia&#8221; is the act of unknowingly putting hand to mouth, usually from a bag or box in front of the television, while reading a book. It can also happen at happy hour, or when you finish the last few bites on your child&#8217;s plate.</p>
<p>&#8220;Resist the temptation to clean yours or anyone else’s plate,&#8221; says Gidus. &#8220;Think about your waistline instead of the food waste.&#8221;</p>

There is only a slight increase over that second year,&#8221; said Dr. Luc Van Gaal (search) of University Hospital in Antwerp, Belgium, who led the company-funded study involving 1,507 severely obese people in Europe. About two-thirds of American adults are overweight or obese; in European countries, one-third to half are. Diet drugs sold now are only for short-term use or have unpleasant side effects that make it tough to stay on them. Acomplia (Zimulti-rimonabant) works in an entirely different way, by blocking a &#8220;pleasure center&#8221; in the brain, leading people to eat less and acting directly on fat cells to prevent weight gain. Company studies suggest it also might help people quit smoking. In a North American study of 3,040 obese people reported last fall, those given the higher of two doses of the drug lost about 19 pounds and kept it off for up to two years, compared with only 5 pounds for those given fake pills.In the new study, those on the higher dose regained some weight in the second year but fared far better than those on placebo. Waistlines in the drug group were 3.4 inches smaller after one year and 3 inches after two.The proportion of people with metabolic syndrome — a collection of unhealthy conditions such as high blood sugar and blood pressure and low amounts of &#8220;good&#8221; cholesterol — went from 42 percent at the start of the study to 21 percent at two years for those on the higher dose of the drug.Dr. Sidney , a former American Heart Association president and cardiology chief at the University of North Carolina, said this was especially important, since 47 million Americans have metabolic syndrome.</p>
<p>However, the drug has side effects: 13.7 percent on the optimal dose reported nausea compared with 5.5 percent taking dummy pills, though researchers said it tended to be mild and short-lived. Rates of dizziness and diarrhea were almost twice as common on the drug.About 19 percent on the higher dose dropped out of the study because a problem occurred, but so did 13 percent in the placebo group. Depression was the reason for discontinuing for 2.8 percent on the higher dose of the drug and 1.6 percent in the placebo group.Dr. Julius Gaardin, a cardiologist at Wayne State University who had no ties to the study or the company, called it and similar ones on the drug &#8220;truly landmark studies in the field of obesity.&#8221;He said &#8220;the safety profile was quite good&#8221; and obesity is such a serious problem that there ought to be higher tolerability of side effects than for drugs for other conditions.Company officials say they are on track to seek Food and Drug Administration approval within a few months.</p>
